The ADHD Symptom Checklist-4 (ADHD-SC4) is a 50-item rating scale, completed by parents and teachers, to screen for ADHD and Oppositional Defiant Disorder (ODD). Items are based on the DSM-IV and worded to be easily understood. The ADHD-SC4 also contains the Peer Conflict Scale (which measures peer aggression) and the Stimulant Side Effects Checklist (to monitor medication). Completed in 15 minutes, it is quick and easy to score.
The Quick Guide contains instructions to start using this practical instrument with a minimum of effort. The Manual contains a description of DSM-IV criteria for AD/HD and ODD; research on reliability, validity, and treatment sensitivity; normative data studies; scoring guidelines, and clinical applications.
The ADHD-SC4 KIT contains the Manual, 50 each of the Checklists and Score Summary Records, 25 each of the Score Sheets for Teacher-Completed and Parent-Completed Checklists and a Quick Guide.
